Our Story

From Historic Hotel to Loving Home

The Logan family didn’t just open a facility — they transformed a piece of Fort Atkinson history into a place where people thrive.

  • 1914 The Black Hawk Hotel opens its doors, becoming a cornerstone of downtown Fort Atkinson — known for fine dining and warm hospitality.
  • 1991 Jack and Sharon Logan transform the historic hotel into Black Hawk Senior Residence — 34 private bedrooms, each with full bathrooms.
  • Today Second-generation ownership. Still locally owned. Still the area's most trusted, most awarded assisted living community.

"We care for our residents for the remainder of their lives."

— Black Hawk Mission Statement

What Families Say

Real words from
real families.

We know you have
questions.

Choosing care for a loved one is one of the biggest decisions of your life. We’re here to help you navigate it — honestly.

Our monthly rate includes housing, meals, and care — no complicated hourly billing. We accept private pay, Medicaid, and VA benefits. Call us for current private pay rates: (920) 563-1480.

Yes. We offer structured, compassionate memory care in a safe, alarmed environment for non-aggressive seniors with dementia or Alzheimer's. Each resident benefits from socialization at every level of functioning.

Absolutely. Resident cats and birds are welcome, and dogs may visit from home. Pets are family — we believe that too.

Our 24-hour awake staff and supervised daily routines significantly reduce fall risk. We also have wheelchair and walker accommodations throughout the building as a State-licensed Class C, N/A facility.

Often faster than you'd expect. We guide families step by step through the process and work hard to make transitions as smooth as possible.

Yes. We accept all medically qualified applicants regardless of ability to pay. Contact Jefferson County ADRC at 920-674-8734 for Medicaid, or Jefferson County Veteran Services at 920-674-7240 for VA funding questions.

Come see what
home really feels like.

Nothing we write here compares to walking through the door — smelling breakfast being made, meeting the staff who know every resident by name.
